HEMEX
Laboratories provides courier services in the greater Phoenix Metropolitan
area. For our customers outside the Phoenix area, we have made arrangements
with couriers in several other areas. Please contact Client Services to
arrange for courier service to your facility or office.
Please observe the following guidelines when shipping samples to HEMEX in order to comply with federal shipping regulations and insure specimen integrity. THESE INSTRUCTIONS MUST BE FOLLOWED IN ORDER TO ENSURE THAT THE PATIENT SAMPLES ARE SHIPPED AND ARRIVE AT HEMEX. These instructions are specific for FedEx. For directions on shipping diagnostic samples via another carrier, please contact that carrier. Please notice there are different
instructions for Frozen, Room
Temperature, and Combination Frozen/Room Temperature
sample shipments.

Each patient's sample(s) must be labeled legibly with: 1. Patient name 2. Collection date 3. Time of draw 4. Sample type (plasma or serum) All samples MUST be submitted
with a corresponding Test Request Form (TRF)
1. Patient name 2. Date of Birth 3. Physician name 4. Location to fax/mail report to 5. Date of blood draw/sample collection 6. Time of blood draw/sample collection 7. Billing information
as requested on the Test Request Form (TRF)
Please see the TRF example following this section as reference. Areas that are shaded MUST be completed to ensure proper processing of billing and patient information. All samples from a given patient are to be submitted in one specimen bag with corresponding TRF placed in the outer pouch of the bag. To be sure the TRF remains dry and with the correct patient specimen, seal the pouch with the outer flap. HEMEX Laboratories will pay
FedEx charges on box sizes up to 9" x 9" x12". Boxes exceeding this size
will be subject to additional charges to you. Please contact Client Services
(800) 999-2568 to obtain appropriate shipping containers for all specimen
types. Please consult the shipping box diagram following COMBINATION FROZEN/ROOM
TEMPERATURE SPECIMENS instructions.
FROZEN SAMPLES
1. Be sure to read and follow REQUIREMENTS FOR ALL SAMPLES. 2. Place sample(s) in the ziplock portion of the specimen bag, expel the air, and seal. 3. Fold the corresponding TRF and place in the outer pouch of the specimen bag. To keep the TRF dry, use the outer flap to seal the pouch. 4. Place the specimen bag in the bottom of a styrofoam shipping container, and surround with three to five pounds of dry ice. The dry ice will maintain its integrity longest if left in one piece and tightly wrapped in paper. Do not break up the dry ice, as exposure to air causes more rapid deterioration. ****NOTE****
5. Fill remaining air space with newspaper or styrofoam filler (such as peanuts or "S") to minimize the deterioration of the dry ice. 6. Close the container and seal by securely taping around the cover. 7. Place sealed styrofoam container inside a cardboard shipping box and seal. 8. Attach shipping label which indicates both the shipper and recipient of the package. HEMEX will pay shipping charges only if FedEx is used as the carrier (see FedEx Airbill Instructions). ****NOTE**** When shipping a sample of a drug being administered, label the drug concentration on the vial, and wrap the vial with insulating material. Ship the vial OUTSIDE the styrofoam container, to avoid freezing. See COMBINATION FROZEN/ROOM TEMPERATURE instructions for more information. ROOM TEMPERATURE
SAMPLES
1. Be sure to read and follow REQUIREMENTS FOR ALL SAMPLES. 2. Place samples inside a styrofoam shipping container and rubber band the two halves together. 3. Place the styrofoam container inside the specimen bag with absorbent pad. 4. Fold the corresponding TRF and place in the outer pouch of the specimen bag. Seal the pouch using the outer flap. 5. Place the specimen bag in the cardboard shipping box and seal. 6. Place the shipping box inside a FedEx Diagnostic Sample Envelope. HEMEX will pay shipping charges only if FedEx is used as the carrier (see FedEx Airbill Instructions). HEMEX Laboratories will pay FedEx charges on box sizes up to 9" x 9" x12". COMBINATION FROZEN/ROOM TEMPERATURE SAMPLES (ISAC PANEL) When shipping multiple samples, some of which are required to be shipped at room temperature, some frozen, please follow these steps: 1. Follow directions 1 - 7 for FROZEN samples. 2. Follow directions 1 - 4 for ROOM TEMPERATURE samples. ****NOTE****
3. Using packing tape, attach the Room Temperature box to the top of the Frozen Styrofoam box. 4. Place both boxes inside the cardboard Shipping box. 5. Fill remaining space around
the Room Temperature box with newspaper
6. Attach shipping label
which indicates both the shipper and recipient of the package.

examples to properly complete a FedEx Airbill for shipment to HEMEX.
FedEx recently began producing airbills that can be affixed directly onto
the shipping box. For your convenience, examples of both types of airbill
have been provided. In order to assure proper shipment, please print legibly
in ink. Should you have questions, please call Client Services at (800)
999-CLOT (2568). HEMEX Laboratories will pay FedEx charges on box sizes
up to 9" x 9" x12". Boxes exceeding this size will be subject to additional
charges to you. Please contact Client Services to obtain appropriate shipping
containers for all specimen types.
Anyone using this information fraudulently will be prosecuted to the fullest extent of the law. Older
type airbill (no adhesive on back)

black and white Dry Ice label must be affixed to the outside of the Shipping
box indicating the amount in kilograms of dry ice enclosed. Do not use
more than 5 pounds (2 kilograms) of dry ice. FedEx may refuse the shipment
if this requirement is not observed. NO PART OF THIS LABEL MAY BE COVERED
UP AND IT MUST BE EASILY READ BY THE CARRIER. FEDEX WILL REFUSE DELIVERY
IF THE LABEL IS NOT COMPLETE OR OBSTRUCTED.
